Annual Cycle of Care

Diabetes changes over time and can lead to complications that affect your kidneys, eyes, feet or heart. The Annual Cycle of Care is designed to help you keep your diabetes care on track. Our trained health professionals perform a number of tests and measurements to identify any problems early on to lower your risk of diabetes-related complications. Your health review information is passed on to your GP for further monitoring. 

The checks performed at this screening include:

  • HbA1C (three month average of your blood glucose levels)
  • Cholesterol
  • Kidney health check
  • Foot examination
  • Blood pressure
  • BMI

You will have the chance to ask any questions you may have about your diabetes. You can also bring your blood glucose testing kit to be checked for calibration and accuracy.

This screening does not replace regular visits with your local diabetes healthcare team. If you’re not sure whether to attend, please discuss with your general practitioner.

This program is for people living with all types of diabetes.
