Annual Cycle of Care Screening

Who is this program for: People with type 1 or type 2 diabetes

Duration: 1 hour

Style: One on one appointment

Diabetes changes over time and can lead to complications that affect your kidneys, eyes, feet and heart. The annual cycle of care is designed to assist you in keeping your diabetes care on track. At the annual cycle of care screening our health professionals will check your 3 month average blood glucose reading, your Cholesterol, blood pressure, kidney health and feet.

Connecting Your Diabetes

Who is this program for: People with type 1 or type 2 diabetes

Duration: 3 hour

Style: Seminar

This informative seminar will provide you with information on connecting with diabetes services in your local area. Topics covered include understanding the NDSS, making healthy eating easy, physical activity, getting your mind around diabetes and the annual cycle of care.

Individual Health Information Session

Who is this program for: People with type 1 or type 2 diabetes

Duration: 45 minutes

Style: One on one appointment

At an individual health information session you will have three 10 minute appointments with a diabetes educator, a dietitian and an exercise physiologist. You will have the opportunity to discuss your general diabetes concerns, be given resources to support your diabetes management. Don’t forget to bring your blood glucose meter which will be cleaned and checked for accuracy on the day.

Living with Insulin

Who is this program for: People currently using insulin

Duration: 3 hours

Style: Small class

If you are new to insulin or have been on insulin for a while and would like a refresher, this program is for you. The Living with Insulin seminar provides practical information about equipment, injecting, home blood glucose monitoring, driving and eating. You will also receive a take home information pack.

Be Well With Diabetes Workshop

Who is this program for: People with type 1 or type 2 diabetes

Duration: 2 hours

Style: Small group workshop

If you are after an interactive program where you can learn to help you better manager your diabetes this program is for you. Broken down into three 45 minute session you will learn about foot care and diabetes complications which can affect the feet, simple exercises you can do at home and learn about practical food choices.

Supermarket Tours

Who is this program for: People with type 1 or type 2 diabetes

Duration: 2 hours

Style: Small group tour, bring your glasses

Make shopping for healthy food easy. Tour the aisles of your local supermarket in a small group lead by a dietitian from Diabetes NSW. On this tour you will learn how to decode food labels, make sense of health claims of packets and get advice on making healthier food choices.

Diabetes Building Healthy Meals

Who is this program for: People with type 1 or type 2 diabetes

Duration: 3 hours

Style: Group workshop

An interactive healthy meal workshop. The Building Healthy Meals workshop will run over 2 hours. Come along and build your knowledge about ways to make recipes healthier, basic label reading and practical tips for the kitchen-including how to save some money. Whilst this isn’t a cooking class, it will set you up with some great ideas to take home to help you enjoy a healthy and varied diet.

Beat It Gym

Who is this program for: People with type 1 or type 2 diabetes

Duration: 8 week program

Style: Small group

Beat It Gym runs over eight weeks. You will gradually be introduced to moderate intensity aerobic, strength and balance based exercises. You will also be provided with education sessions on healthy living topics. The sessions are fun, safe and supportive with the exercises specifically tailored to suit your individual health and fitness levels.

Moving for Health

Who is this program for: People with type 1 or type 2 diabetes

Duration: 1 hour

Style: Group session, wear comfy exercise clothes

A fun low intensity movement session. Learn how to do simple movements while sitting or standing that can help with your overall health. Exercise is shown to have a positive impact on diabetes management and this session will show you how easy it can be and give you a chance to learn the basic movements from an Exercise Physiologist. At the end of the session you will receive a FREE booklet with exercise instructions and a flexible rubber band (called a TheraBand) to take home with you to keep moving at home. This session is suitable for everyone regardless of your age or how mobile you are.

DESMOND (Diabetes Education and Self-Management for Ongoing and Newly Diagnosed)

Who is this program for: People diagnosed within the last 12 months

Duration: 1 day

Style: Small group workshop

DESMOND will empower you to take control of your diabetes care. You will be given practical information about food choices, physical activity and medications in a small group environment. Our specially trained health professionals will lead the day, answer any questions you may have and provide you with valuable support to help you live your life with diabetes. This program is also a chance to meet other newly diagnosed people and share your experiences. You will receive a take home information pack.
